This is to bring to your kind notice that I will be closing my account number 3088 1234 5678 0009 by the end of the month with ABC credit company.

I already paid my account balance with check number 3811 last December 28, 2003. All my fiscal responsibilities with this credit card account have been fulfilled.   Please close my account and include a notation in the report to the credit bureaus that the account was “closed by request of cardholder.”

Please send me written evidence of the closure of my account, in good standing and at my request. Also, I am enclosing my destroyed credit card.
