Dear Ms. Villanueva:

I am writing to inform you of my recent financial hardship.  As you already know, the above referenced account is past due.

My wife has had a recurring medial problem and we are now spending an additional P15,000.00 per month on prescriptions that we did not anticipate.

With my additional unexpected expenses, I am requesting to be considered for a payment plan program that would allow me to keep making lower payments until I can resume normal payments or the debt is paid in full.

Please find the enclosed xerox copies of personal  documents that will support my financial difficulty at the moment.
